Couples Therapy for When One or Both Partners Are Wired Differently
Starting From How You're Each Wired, Not What's "Wrong"
Often, neurodivergent couples therapy assumes a neurotypical partner is paired with a neurodivergent one. In reality, ADHD and autism can show up in either partner or both (sometimes called AuDHD), and one-size-fits-all couples counseling often misses what's actually happening between you. This page starts from a different assumption: one or both of you may be wired differently, and that's the starting point for real communication, not something that needs to be fixed or explained away.
Sessions are led by Mark Wilson, LMSW, a licensed clinician who also works with families through our ADHD and autism parent coaching programs, bringing a neurodivergence-informed lens to couples therapy as well.
Neurodivergent Couples Therapy Built Around Communication Strategies
Rather than general relationship advice, sessions focus on concrete communication strategies and coping skills tailored to how each partner processes the world, whether that's sensory sensitivities, emotional regulation, or differences in how you each perceive and respond to conflict.
What Is Neurodivergent Couples Therapy?
When one partner reads a pause as calm, and the other reads it as distance, small misunderstandings compound fast. A neurodivergent couples therapist helps you see those moments for what they are: two different neurotypes trying to connect, not one partner being difficult.
This draws on researcher Damian Milton's double empathy problem, a framework describing how people with different neurotypes may experience empathy differently, express it differently, communicate it differently, and interpret it differently from one another.
A communication breakdown isn't a sign that one partner lacks empathy; it's a sign that two different ways of experiencing and communicating haven't yet found a shared language. Different doesn't mean deficient, and the focus of therapy is understanding that disconnect together, not diagnosing which partner is "wrong."

frequently asked questions
-
Traditional couples therapy often assumes both partners process communication and conflict the same way. Neurodivergent couples therapy accounts for real neurological differences from the start.
-
Common stressors include mismatched communication styles, sensory sensitivities that create friction, one partner carrying most of the emotional labor, and cues or intentions that get lost between how one partner sends a message and how the other receives it. These aren't relationship failures. They're relational patterns shaped by neurological differences.
-
If you or your partner have ADHD, are autistic, or both (diagnosed or not), and you find yourselves stuck in the same disconnect around communication, intimacy, or daily routines, neurodivergent couples therapy can help you understand why and build new patterns together.
-
Sessions focus on communicating effectively across different neurotypes: clarification of misunderstood cues, structured emotional check-ins, nervous-system regulation tools, and collaborative problem-solving built around how each partner's brain actually works.
-
That distress is real, and it deserves to be named and taken seriously. Persistent communication disconnects can leave either or both partners feeling lonely, misunderstood, exhausted, or disconnected over time. Naming that relational burnout and emotional isolation, without assigning blame to either partner's neurotype, is often the first step toward being heard.
-
Not fundamentally. Whether you're both autistic, both ADHD, or a mixed-neurotype couple, the work centers on your specific relationship dynamics rather than a diagnosis-first approach. Resources like the Association for Autism and Neurodiversity (AANE) offer additional perspective for neurodiverse relationships.
